David Mzee, un patient rendu parapl\u00e9gique lors d\u2019un accident de sport ayant provoqu\u00e9 une l\u00e9sion partielle de sa moelle \u00e9pini\u00e8re, quittait sa chaise roulante pour se mettre \u00e0 marcher avec l\u2019aide d\u2019un d\u00e9ambulateur. La r\u00e9activation de la moelle \u00e9pini\u00e8re avec des stimulations \u00e9lectriques donnait les premi\u00e8res preuves de sa pertinence.<\/p>\n<p>Trois ans plus tard, une nouvelle \u00e9tape est franchie par les \u00e9quipes de Gr\u00e9goire Courtine, neuroscientifique et professeur \u00e0 l\u2019EPFL, et Jocelyne Bloch, neurochirurgienne et professeure au CHUV. Grace au d\u00e9veloppement de nouveaux implants optimis\u00e9s pour stimuler la r\u00e9gion de la moelle \u00e9pini\u00e8re qui contr\u00f4le les muscles du tronc et des jambes et d\u2019un nouveau software int\u00e9grant de l\u2019intelligence artificielle, trois patients ayant souffert d\u2019une l\u00e9sion compl\u00e8te de la moelle \u00e9pini\u00e8re sont \u00e0 pr\u00e9sent capables de marcher en dehors du laboratoire. \u00abNos algorithmes de stimulation continuent \u00e0 imiter la nature, explique Gr\u00e9goire Courtine. Les nouveaux implants souples que nous pla\u00e7ons sous les vert\u00e8bres au contact de la moelle \u00e9pini\u00e8re sont capable de moduler les neurones qui r\u00e9gulent l\u2019activit\u00e9 de groupes musculaires pr\u00e9cis. On peut ainsi activer la moelle \u00e9pini\u00e8re comme le cerveau le ferait naturellement pour tenir debout, marcher, faire du v\u00e9lo, de la natation, etc.\u00bb<\/p>\n<p><strong>Un bouton pour activer les s\u00e9quences motrices<\/strong><br \/>\nEn cette froide journ\u00e9e de d\u00e9cembre, des giboul\u00e9es de neige s\u2019abattent sur le Grand-Pont, \u00e0 Lausanne. La bise glac\u00e9e ne d\u00e9courage pourtant pas Michel Roccati, un patient italien qui souffre d\u2019une parapl\u00e9gie compl\u00e8te suite \u00e0 un accident de moto survenu 4 ans plus t\u00f4t. Il a lui aussi re\u00e7u le nouvel implant plac\u00e9 contre sa moelle \u00e9pini\u00e8re au CHUV par la neurochirurgienne Jocelyne Bloch.<\/p>\n<p>Autour de lui, l\u2019\u00e9quipe du centre .NeuroRestore s\u2019affaire. Deux petites t\u00e9l\u00e9commandes sont fix\u00e9es sur un d\u00e9ambulateur. Une tablette envoie, sans fil, les commandes de stimulation au pacemaker qui est situ\u00e9 dans l\u2019abdomen de Michel, et qui ensuite relaie les stimulations \u00e0 l\u2019implant m\u00e9dullaire pour permettre \u00e0 Michel de se lever. Cramponn\u00e9 \u00e0 son d\u00e9ambulateur, il fait la d\u00e9monstration du syst\u00e8me. Une pression sur le bouton de droite, conjointement \u00e0 sa volont\u00e9 d\u2019activer ses muscles, et sa jambe gauche se fl\u00e9chit comme par magie. Puis se repose, quelques centim\u00e8tres plus loin. Michel actionne alors le bouton de gauche et sa jambe droite fait un pas. Il marche ! \u00abLes tout premiers pas ont \u00e9t\u00e9 vraiment incroyables, inesp\u00e9r\u00e9s ! t\u00e9moigne-t-il. Je m\u2019entra\u00eene \u00e9norm\u00e9ment depuis plusieurs mois. Je fixe mes objectifs. Je peux m\u00eame monter et descendre des escaliers. Je pense pouvoir franchir 1 km d\u2019ici l&#8217;\u00e9t\u00e9.\u00bb<\/p>\n<p>Deux autres patients suivent avec succ\u00e8s le m\u00eame protocole,\u00a0<a href=\"https:\/\/www.nature.com\/articles\/s41591-021-01663-5\">d\u00e9crit aujourd\u2019hui dans Nature Medicine<\/a>. \u00abLa cl\u00e9, ici, a \u00e9t\u00e9 de pouvoir ins\u00e9rer un implant plus long et plus large, avec des \u00e9lectrodes dispos\u00e9es de mani\u00e8re \u00e0 les faire correspondre pr\u00e9cis\u00e9ment aux racines nerveuses de la moelle \u00e9pini\u00e8re qui nous permettent d\u2019acc\u00e9der aux neurones qui contr\u00f4lent les muscles \u00bb, pr\u00e9cise Jocelyne Bloch. Cela permet davantage de s\u00e9lectivit\u00e9 et de pr\u00e9cision dans le contr\u00f4le des s\u00e9quences motrices associ\u00e9es \u00e0 chaque activit\u00e9.<\/p>\n<p><strong>En un seul jour<\/strong><br \/>\nUn entra\u00eenement pouss\u00e9 est bien s\u00fbr n\u00e9cessaire pour que les patients gagnent en mobilit\u00e9. La r\u00e9cup\u00e9ration n\u2019en est pas moins spectaculaire : \u00abEn une seule journ\u00e9e apr\u00e8s l\u2019activation de leur implant, nos trois patients pouvaient se lever, marcher, p\u00e9daler, nager et contr\u00f4ler des mouvements du tronc, pr\u00e9cise Gr\u00e9goire Courtine. Ceci gr\u00e2ce \u00e0 des programmes de stimulation sp\u00e9cifiques \u00e0 chaque type d\u2019activit\u00e9, qui peuvent \u00eatre s\u00e9lectionn\u00e9s \u00e0 la demande sur la tablette, et ensuite g\u00e9n\u00e9r\u00e9s par le pacemaker implant\u00e9 dans l\u2019abdomen.\u00bb<\/p>\n<p>Aussi impressionnants que soient les r\u00e9sultats imm\u00e9diats, c\u2019est toutefois apr\u00e8s quelques mois d\u2019entra\u00eenement que les progr\u00e8s se sont r\u00e9v\u00e9l\u00e9s les plus spectaculaires. Un programme d\u2019entra\u00eenement \u00e0 l\u2019aide des programmes de stimulation a permis aux patients de regagner de la masse musculaire, d\u2019augmenter leur autonomie de mouvement, et de renouer avec certaines activit\u00e9s sociales \u2013 partager une boisson debout \u00e0 un bar, par exemple. Avantage consid\u00e9rable, gr\u00e2ce \u00e0 la miniaturisation des \u00e9quipements, ces entra\u00eenements peuvent se d\u00e9rouler en ext\u00e9rieur et non plus seulement dans un laboratoire.<\/p>\n<p>\u00abNous avons d\u00e9montr\u00e9 une fois de plus la pertinence de notre approche, souligne Gr\u00e9goire Courtine.
